2012 Minority Business Opportunity Awards Nomination Form

 The Business Development Connection of the Greater Shreveport Chamber of Commerce is seeking nominations for the Annual Minority Business Opportunity Awards Luncheon which will be held on February 16, 2012 at the convention center.  The theme for this years’ award is “Celebrating Excellence and Courage”.  The top three nominees for each category and recipients will be recognized at the luncheon.  The categories and qualifications are listed below:

 Lifetime Achievement Award: An individual who has consistently exemplified the highest standard of professional excellence  throughout his/her career; has dedicated his/her life to providing service to the community; has a record of accomplishments; and has a reputation that is widespread.

Small Business Owner of the Year:  An individual, who owns, operates and bears the principal responsibility for operating a small business.  Partners may be nominated as a team providing the number of individuals in the team does not exceed four (4).

Emerging Business of the Year:  A business that is poised for future growth and has carved a niche for itself in the business community.
